Sourcing Appointment Form Instructions - Note: If you are using a Mac you can only use Mozilla's Firefox Browser or Internet Explorer. Apple Safari Browser will not work with this form.
 
Please review carefully before scheduling an appointment and follow instructions extremely carefully. Mistakes in dates and times can be extremely detrimental to calendar so please check that your entries conform to instructions below.
 
Location: 202 W 40th St 902 (9th Fl) NY NY 10018 (just West of 7th Ave) 
 
1. Last name - if you have previously filled out our on-line Sourcing Registration Form you will find your last name here by scrolling up or down. Please select last name. If your name is not here please go to our sourcing link at http://gidc1.org/sourcingregistration.aspx and fill in all information. You may then go back here and schedule an appointment
2. Full Name - enter Full Name
3. Start Time - Sourcing Appointments are scheduled on Mondays and Thursdays only between the hours of 10:00 am and 5:00 pm. If you are scheduling an appointment on Monday or Thursday during these hours please complete the Start Time by filling in month/date/year and time in following format 7/10/2009 1:00 pm. Your appointment will magically appear on my calendar. If you absolutely can't make it on a Monday or Thursday due to the fact your coming from out of town please call 212 366 6160 ext 108 to inquire about alternatives.
4. End  Time - In order to complete as many sourcing appointments as possible please fill in correct end time in same format and use 1/2 hour window. Ex. Start Time 7/10/2009 1:00 pm End Time will be 7/10/2009 1:30 pm.
5. Description - Please reference specifics on sourcing needs and any additions to original sourcing registrations here
6. What should you bring for the sourcing appointment?  Samples, Sketches, Fabrics, Retail and Wholesale targets
